In Ireland, we only have two lane motorways so the fast lane is the overtaking lane and your not allowed stay in it (although a lot of people do).
Once you have overtaken slower traffic, you should pull back into the slow lane so faster traffic can overtake you.
Most of our other roads are single lane only and in cities one way streets because of how old all the streets and roads are and the sheer amount of them as Ireland has more roads for its size than any other country in the EU.
I can't stand when people sit in the passing lane and create a road block. It contradicts the whole flow of traffic, especially on busy days. Especially out here in Mass, where the massholes flow like beer.
In the UK, the rightmost lane is always referred to as the "fast lane", even though it's technically called "lane three".
It would be interesting to see different speed limits imposed for each lane. Either that or remove the restriction on passing on the inside. (It's illegal to overtake on the left, for some reason.)

that's a separate issue, "the HOV lane" which is a special purpose lane they choose to put on the left.

In the United States, there are some areas where exits are on the left. In this case, the right lane becomes the fast lane.
