I would say that 90% of the time that the flash would go off would be counter-productive for me. At least half the time the subject isn't in range of the flash. The rest of the time it is just more polite to not use a flash.
It annoys me off to see people use a built-in flash to take a picture of something 50ft. away at night.

Every major camera brand has a button with the flash icon (example for an Olympus camera).
The problem arises when people sometimes want to turn it on and they don't know how. Probably they aren't aware of that symbol.
Correction on my above comment:
*"turn it on" = turn it off
I could see a situation where someone wanted to turn it off, and it was too dark to see the symbol.
